Reduce your chances of being readmitted and increase your chances for a healthy recovery by planning early. Take steps as soon as possible during your stay to plan for a successful transition from the hospital.
To begin, ask to speak with your discharge planner, and review the following:
Try the teach-back method—repeat back what you hear the discharge planner say to make sure you understand the details correctly.

Not Ready to Leave?
You have the right to appeal your discharge if you don’t agree with the decision that you are ready to leave the hospital. Speak with your doctor or nurse, and share your concerns. You may also need to reach out to Medicare, Medicaid or your insurance company.
Be sure to get copies of your medical records and test results before you leave. You will need to submit a medical records authorization form to get copies of them. Visit www.providence.org/about/medical-records-authorization/medical-records-providence to download the form, then submit it to the Medical Records Department in the hospital or fax it to 310-303-5469.
